Sultana Athletic Physical and Clearance Process:

In order for student athletes to be cleared to participate in athletics they must have a current physical on file (new each school year, not calendar year) and have the physical packet filled out and signed by both a parent or guardian and the student. Physical packets can be picked up in the Athletics Office or Athletic Training Office. Once the physical packets have been turned in and approved, the student athlete will be given an "OK to Play" card that must be given to the Head Coach.
